I think its a case of getting too comfortable with moving pebbles around, and a moments carelessness and the reality is it is not a pebble, neither is it a Formula 1 race car.

not really , bad training , long hours ,longer shifts . boredom of doing repetitive tasks ,that would be closer to the real answer.
