That is what happens when you tail gate.
Always keep at least two car lengths from the vehicle in front of you, even when doing 5 mph.
If you feel sleepy, always pull into a safe place for a rest. The middle of an operating coal mine fails to qualify as a safe place for a nap.
If you are driving a vehicle and another vehicle is tail gating, do not slam on the brakes and inconvenience the other driver. Slow down to 4 mph and allow the other vehicle to over take you safely at 5 mph.
Always check your reversing lights and reverse warning devices before proceeding in a heavy vehicle.
When driving always pay attention to the road in front of you. Do not get distracted by kangaroos copulating and make rude noises or shout ” go boy, go!” from the car window. Inattentiveness while driving can lead to dismissal.
Do not abandon company vehicles around the company site. Severe penalties apply if the vehicle must be recovered or is not returned with a full tank.
Incident reports must be completed in full. If vehicles have damage mark the area on the return form. If a vehicle experiences a mechanical breakdown, advise the Maintenance Manager. “She’s fucked” is inadequate at too many levels.

First of all buddy was not tailgating…..he or she simply parked behind a D11 to hide from boss and smoke some weed or nap. When the the D11 started backing up, driver bailed (first good decision that day) and cut finger on door handle in panic. Had driver remained with ship, next of kin would be needed.
